Trainee Evaluations

I am looking for some information regarding Trainee evaluations. I am in the process of revising the trainee evaluation we currently use to evaluate the learners performance during training. I currently work in a "Call Center like" atmostphere and we conduct new hire training. During training we evaluate the individual learner on 1) comprehension, 2)participation/interest, 3) and the trainers observations of the trainee during class. I am looking for some information that will assist me in revising the questions on the evaluation form. I have a level 1 evaluation that my company uses for the course but I can not find much information on the trainee evaluation process. Can any one assist me? Of course the trainee's managing supervisor is responsible for ongoing job performance but we do trainee evaluations to present to the supervisor on our observations and the trainees performance during training. Trainers are not customarily responsible for the performance evaluations but in my position we as trainers are faced with having to do so. It is also something that the managing supervisors utilize.
